I think you're right that the NCAA does not have direct power to cause an
AD to be fired. However there is more than one way to address a problem
and certainly the folks in Indianapolis could have made known (and perhaps
did) their extreme displeasure with the inaction of the AD in question. My
suspicion is they did not, in part because Smith has been very active
within NCAA governance circles. I don't know if they can issue a show
cause order for an AD and I don't have an up-to-date rule book handy.
Theoretically i suppose there would be no reason they couldn't since the
actions of an AD fall under the purview of the NCAA. That said, I can't
recall a case where an AD was sanctioned, even in the face of the dreaded
lack of institutional control, which is one step further than the finding
against OSU.
