We went through that last season when ESPN put a Louisville game on 
ESPNU and the local fans went nuts because the our cable system, 
Insight, didn't carry it at the time.  There was so much pressure that 
they found a way to carry it temporarily so that the game would be 
available locally and, miracles of miracles, it's been on the digital 
sports tier ever since.  ESPN is using the schedule to pressure local 
cable systems into carrying the U and you will see more and more big 
name schools have the odd game there as Bristol attempts to pressure 
systems to carry it.  Expect it for the next year or two and then it 
should be available in almost all systems and ESPN will move on to 
playing little games like that for some new channel.
