I think there would be a great deal of peer pressure to participate in the hazing or "initiation" and although the upperclass players aren't the coaches, I think you make far too little of the ramifications of a player being an outcast from the rest of the team. The freshman who refused to participate would be an outcast not just with respect to the upperclassmen, but with respect to her freshman teammates, as well.
Having said that, I have to believe that the coaches had some knowledge that this type of thing goes on, and although they didn't sanction it, they didn't do anything to prevent it, either. Some of the players must have very close relationships with the coaches, plus, the coaches were once players themselves. It's inevitable that they would know that this kind of thing was going on, even if they were in the dark about the details.
Then again, I think the jury's out on the gravity of the situation, but even assuming the best, it's still extremely embarrassing.
Regarding how the pictures got out, I think that most photo-sharing sites aren't very secure. With the right link, the photos are usually accessible to anyone. I'm not sure that everyone realizes that.
