[Husker] Field turf issues???

On the Huskerpedia Game Page, in the fourth article from the Pittsburgh  
Tribune-Review, Rob Biertempfel reports that Pitt player Mike Phillips'  foot got 
stuck in the turf (and then twisted) which caused the broken  ankle.   
Was that just a fluke or have there been other injuries?   I am  wondering 
how you get your foot stuck - are there holes that didn't get  covered?  Wrong 
size cleats for the surface?  Any information will be  appreciated.
