The blue cat 5 wire almost certainly comes in from outside as most of the feeds from that wire are connected to the "from demarcation" connections on the Leviton board. The blue and white wires from that cable go into the splitter and then (1) feed the cable that goes to the AT&T router and (2) come out the telephone outputs on the front of the splitter into the "line in" inputs on the Leviton board. I didn't think it would because it is disconnected a the other end where it went into the panel, but I tried it anyway (still attached to the terminal strips which I removed from the HAI board).
